Federal and State Required Information
FAMILY RIGHTS:According to the Family Educational Rights and Privacy Act (FERPA) “An educational agency or institution may disclose directory information if it has given public notice to parents of students in attendance and eligible students in attendance at the agency or institution.”The district may disclose any of these items designated as “Directory Information” without prior written consent, unless notified in writing to the contrary by September 1 of each year.Notification must be made to the school office in the building in which the student is in attendance.Parents may ask to remove all or part of the information regarding their children that they do not wish to be available to the public without their consent.
“Directory Information” may include name, address, phone number, dates of attendance, grade level, photographs, video taped images for publication in such places as the yearbook, newspaper, school photographer, local news media and graduation, participation in officially recognized activities/sports and degrees, honors and awards received.
FERPA also affords parents the right to inspect and review the student’s education records within 45 days of the day the school receives a request for access.The school official will make arrangements for access and notify the parent of the time and place where the records may be inspected.
Parents may ask the school to amend a record they believe is inaccurate. They should write the school principal, clearly identify the part of the records they want changed, and specify why it is inaccurate.If the school decides not to amend the record as requested by the parent, the school will notify the parent of the decision and advise them of their right to a hearing regarding the request for amendment.Additional information regarding the hearing procedures will be provided to the parent when notified of the right to a hearing.
ASBESTOS:In compliance with the AHERA regulations, our school facilities have been inspected and are under a management plan to deal with the existing materials containing asbestos.Regular inspections are held according to law.A copy of the Asbestos Management Plan is available for review during the regular office hours in our administrative office.

SECTION 504 OF THE AMERICANS WITH DISABILITIES ACT:Lockwood School District #26 does not discriminate on the basis of disability in admission to, access to, treatment in, or employment in its programs and activities.
IMMUNIZATIONS:Parents are required to supply updated records to include immunizations, recently identified allergies and emergency contact information. For those students with milk allergies or intolerance, we need an updated doctor’s statement each year in order to provide an alternative to milk.
