Middle School General Information - 2005-06
Each year all schools have to submit an immunization report to the state health deparment. We have to document all 6th - 8th grade students who have received a tetanus booster since the one they received with their kindergarten shots. State Law is being revised and a tetanus booster will be mandatory for 7th grade students next year.
If your child has received a tetanus booster in the
last 5 years, please send a copy of it to the School Nurse. It
will be on your immunization record as a DT or Td. We will make
sure that it is recorded on your child’s health record.
Broken or bent glasses? Please contact
the nurse if your child has glasses that need to be repaired
or adjusted and you need assistance.
Health concerns: Please contact the nurse if your child develops a health problem or allergy that we need to be aware of. We work hard to maintain a healthy environment at school for all of our students.
