Any employee or volunteer seeking assignment at Lockwood School must have this form which authorizes a complete background investigation. This is necessary to protect the safety and welfare of the children in the Lockwood School District. The cost of the background check with fingerprints is $29.25.

*All employees, including substitutes, must have a TB test.
